Rules & Regulations:
  1. Contestant must be at least 18 years of age.
  2. Signature on this form to be used as photo release.
  3. Frontal nudity will be cause for immediate disqualification.
  4. Shoes and/or some sort of foot covering must be worn at all times.
  5. Contestants will be judged on sexiness, creativity & difficulty.
  6. Deadline for entering is Oct. 24th at 6:00PM.
  7. A maximum of 30 contestants will be allowed on a first come first serve basis.
  8. A $20.00 entry fee is due upon registration and must accompany this form.
  9. Judging of event will begin at 10:30PM.
  10. First prize is $1,500.00.
